POSITION SUMMARY The Wellness Concierge is responsible for supporting residents in scheduling medical appointments, coordinating care services, and assisting with transportation arrangements. This role serves as a resource to ensure residents receive prompt attention to their healthcare needs and works closely with the Health and Wellness Director, care teams, and external providers to facilitate seamless care coordination. KEY RESPONSIBILITIES Care Coordination Schedule and document medical, dental, and therapy appointments for residents. Collaborate with the Health and Wellness Director and the pharmacy to coordinate medication orders and prescription refills. Assist residents and families in navigating healthcare resources. Communication and Documentation Maintain accurate records of appointments, care updates, and communications in residents' files. Notify care teams and leadership of significant updates related to residents' health or care needs. Transportation Support Arrange transportation for medical appointments and outings to ensure timely and efficient service. Serve as a liaison between residents and transportation vendors, resolving scheduling conflicts when needed. Resident Support Act as a friendly and reliable point of contact for residents seeking assistance with care-related matters. Address resident and family inquiries promptly, providing updates or resolving concerns as required. Compliance and Team Collaboration Ensure all scheduling and care coordination activities comply with community and state regulations. Participate in care team meetings to discuss resident needs and contribute to the development of care plans. Additional Duties Support special projects and events related to resident health and wellness. Adhere to Cogir Senior Living policies and procedures. Perform additional tasks as assigned. Requirements CANDIDATE QUALIFICATIONS Education: A High school diploma or equivalent is required. Post-secondary education in healthcare administration or a related field is preferred. Experience, Competencies, and Skills: At least 2 years of experience in customer service or an administrative role, preferably in assisted living or healthcare settings. Familiarity with scheduling systems, electronic medical records (EMR), and basic medical terminology is preferred. Exceptional organizational and multitasking abilities. Strong interpersonal and communication skills, with the ability to engage professionally with residents, families, and healthcare providers. Proficiency in Microsoft Office Suite and scheduling software. A current Tuberculosis skin test or chest x-ray is required. Ability to maintain confidentiality and exercise sound judgment in decision-making.
