We are a compassionate and dedicated Home Health & Hospice Care Agency seeking a reliable and detail-oriented Intake / Receptionist to join our office team. This role is vital in ensuring smooth patient admissions, accurate medical recordkeeping, and professional front-office support.

Key Responsibilities:
*Greet and assist patients, families, and visitors in a professional manner
*Gather and input patient demographic and personal information
*Process patient admissions and discharges accurately and efficiently
*Maintain and file medical records in compliance with HIPAA and agency standards
*Distribute medical charts and documents to appropriate departments
*Assist with audits, investigations, and departmental reporting
*Respond to phone calls, emails, and other clerical tasks
*Ensure all medical documentation is complete, accurate, and confidential
*Provide administrative support to the nursing department

MINIMUM QUALIFICATIONS:
*High school diploma or equivalent qualification.
*Advanced understanding of medical terminology and administration processes.
*Proficient in information management programs and MS Office.
*Outstanding communication and interpersonal abilities.
*Strong attention to detail with excellent organizational skills.
*Have satisfactory references from previous (or current) employers and/or nursing school, and professional peers.
*Have excellent observation, good clinical judgment, and good oral and written communication skills.
*Is self-directed with the ability to work with little supervision; has good organizational skills.
